The files available for download here are done in Microsoft Word 97 (*.doc) format or in Adobe Acrobat (*.pdf) format.   

If you have a word processing software package that supports Word 97 document format, you may download those forms and enter information required on the applications supplied but you may not alter the actual forms otherwise the Bureau will not accept them.  

If you do not have Word 97 supported software, you can still download and print out hard copies of the blank forms by using Adobe Acrobat (a free software application) and printing the forms on a printer.

At this time, the Bureau does not have the capability to accept completed forms electronically.  Applicants will be required to follow the instructions on the applications with regard to how the forms are filed for Bureau action.
